When retrieved memory influences an agent's output or action, the source record is logged and traceable under MON-01. This traceability supports explanation (OUT-13), contestability (OUT-14), and post-incident analysis. Without it, an assessor cannot determine whether a given agent output was influenced by retrieved content, or which content influenced it.

## Maturity

- L1 Ad hoc: Controls exist informally; documentation is incomplete; no consistent review.
- L2 Documented: Controls are written down; ownership is assigned; review cadence is defined.
- L3 Operated: Controls are followed in practice; evidence is collected; deviations are tracked. Published criterion for DAT-13: Source records are logged for all retrieval-influenced outputs; traceability has been verified through sampling; the trace chain from output to source record is reconstructable for audit.
- L4 Measured: Control effectiveness is measured against documented thresholds; trends are reported.
